Submission Composition

Investigators or sponsors who wish to submit a research proposal for consideration by the Canadian Neuromuscular Disease Registry (CNDR) must submit the following documents to the CNDR Project Manager:

  • CNDR Research Proposal Submission Form
  • Copy of Letter of Approval from Ethics Review Board OR Copy of Application and Expected Date for a Decision*
  • Copy of Investigator's Brochure for Trial/Study if applicable
  • Copy of Research Study Protocol
  • Copy of Consent Forms/Withdrawal Forms as applicable
  • Study Budget
  • Proof of Study Funding (e.g. Letter of Commitment)
  • Applicable Regulatory Documents
  • Principal Investigator CV
  • Adverse Event Reporting (if any exists)
  • Copy of Previous Peer Review if available**

* Please note that applications that have not been approved by ethics can be pre-reviewed for completeness of submission by the CNDR but will not be considered for full review until ethics approval is received.

** Must be included if expedited review is requested.

Submission Timelines

Research proposal submission must allow adequate time for review by the CNDR. In general, research proposal review will be conducted on a case by case basis. The desired timeline should be indicated on the Research Proposal Submission Form.
